Sleep Laboratory Referral
In accordance with Ontario regulations, a patient may not self-refer to a sleep laboratory. If you have a patient that requires a sleep consultation or sleep diagnostics, simply complete the attached requisition and return to the Sleep Diagnostics Centre, Barrie Sleep Disorders Clinic or York County Sleep Disorders Centre.
