In accordance with Independent Health Facility standards, a patient may not self-refer to a sleep laboratory. If you have a patient that requires a sleep consultation or sleep diagnostics, simply complete the attached requisition and return to the Sleep Diagnostics Centre or Barrie Sleep Disorders Clinic.
